In-Home Services
In-Home Services are comprehensive, community home-based mental health treatment and case management services designed to support children/youth residing in the least restrictive environment. Service provision is tailored to meet the individual needs of each client/family, with frequency and duration of services based on the needs of the client/ family. Services are provided to families, in which the client is at-risk for out-of-home placement, families in which the client is returning to the home following out-of-home placement, families for whom stressors are negatively impacting the client and family functioning, and clients working towards independent living.

Program Description
In-Home Services include the following components: initial assessment; individual and family counseling; behavioral support; life skills; parenting; communication skills; training; case management activities; coordination with other services; emergency response and support services 24/7. In conjunction with the referral information the initial assessment will identify characteristics and individualized service needs, frequency, intensity, and duration.

Goals
The paramount goal of Hallmark's In-Home Family Services is to enable children/youth to live safely in their homes and community. In addition, one or more of the following goals are to be accomplished:

  • To enhance individual and family functioning and well-being within the context of their home, community, and culture;
  • To strengthen, stabilize, and preserve families;
  • To prevent, reduce, or eliminate behaviors that place a client, family, or community at-risk
  • To reunite youth with their families and reintegrate client into the community following out-of-home placement;
  • To promote safe, stable, and nurturing family environments while maintaining or strengthening family connections; and
  • To assist parents in improving their parenting skills and identifying their parental strengths while supporting parents in their efforts to care for their children.

Discharge Planning
Discharge planning is an integral part of the In-Home Family Services program. The treatment team, upon the admission of the client, initiates the discharge planning process and determines the criteria for successful discharge.
